All services are provided in the patient’s home.

•The home can be the family home, a community residence, a group home, a supportive apartment or family care.
• Care is usually provided until the medical condition has improved or stabilized. However, each case is unique and in certain instances the care can last for an extended period of time.
• Services are provided up to seven days per week in Manhattan, Brooklyn, Staten Island, Queens, the Bronx and Long Island.

YAI Home Health Services offer professional nursing and rehabilitation services designed to support people with developmental disabilities at home.
A registered nurse can provide:
• Post-hospitalization follow-up and monitoring
• Medication preparation, teaching and supervision
• Insulin injection and pre-fill
• Other injections as ordered by the physician
• Wound, pressure sore and other skin care
• Ostomy care
• Catheter care
• Nurse monitoring when health is unstable
• Other appropriate services as ordered by the physician

Physical therapists, occupational therapists, speech and language therapists can provide:
• Therapy to promote recovery from fractures and other conditions of the bones and muscles
• Short-term therapy to teach individuals to be more independent in the home
• Therapy designed to prevent contractures and promote comfort
• Therapy to teach individuals to speak or swallow following a stroke or other neurological condition
• Therapy to teach the consumer and family exercises necessary to maintain improvement

Home health aides provide:
• Support for the patient and the family through assistance with feeding, toileting, bathing, grooming and other activities of daily living

How to receive home health services

You have the right to choose an agency with an expertise in meeting the specialized medical needs of people with developmental disabilities. Please contact us if:
• You feel there is a medical need for home health services
• There is an anticipated or actual admission to a hospital

YAI Home Health Services will ensure that services are in place at the time they are needed most. Referrals can be made by the patient or person with a disability, family member, physician, case manager, residence manager, discharge planner or other professional. YAI will then contact the physician and, if appropriate, obtain medical orders for services. 

If there is a medical need that requires a registered nurse or a therapist, you should call us (in New York City) at 212-255-4040 or (Long Island) 631-858-9856. If you are eligible for care, we will arrange for a nursing assessment.

Plan in advance: Contact us to begin the referral process once the patient is admitted or scheduled to be admitted to a hospital. Do not wait to be discharged.
