A Designated Patient-Centered Medical Home

PHC was designated a Patient-Centered Medical Home (PCMH) by the National Committee of Quality Assurance. In contrast to the traditional medical model, the PCMH is designed to strengthen the physician-patient relationship with coordinated care and a long-term healing relationship.

On September 30, 2009, the National Council on Disability submitted an extensive research report to President Obama that cited Premier HealthCare as one of only four examples of effective health care programs for people with developmental disabilities nationwide. 

With medical professionals who are specially trained to work with people with disabilities and physicians affiliated with major hospitals in the New York metropolitan area, PHC serves as a medical home for people with disabilities, who receive the comprehensive, coordinated health care services that they require, in comfortable practices located in the Bronx, Brooklyn, Manhattan and Queens.
