In order for a child to return to in-person learning at a New York City Department of Education (NYC DOE) location, the family must submit the consent form for in-school COVID-19 testing. Mandatory in-school testing will happen weekly for a random selection of staff and students in all reopened school buildings. If you have already submitted consent, the DOE is encouraging families to do so again to ensure their student has the latest form on file. Exemptions will only be granted in certain limited cases. Additonal information on exemptions, a copy of the consent forms, and information on how to submit the forms can be found on the DOE's website. The consent form is available in ten languages, including English, Spanish, and Bengali.
