These days, health care is big news. One of the most positive developments on the medical front is the Patient-Centered Medical Home (PCMH) movement. A medical home is more than just a place; it is a comprehensive approach to providing care. The American Academy of Family Physicians, the American Academy of Pediatrics, the American College of Physicians and the American Osteopathic Association have defined a Medical Home as “an approach to providing comprehensive primary care... that facilitates partnerships between individual patients, and their personal providers, and when appropriate, the patient’s family.”

In other words, it’s a team-based model of care led by a personal physician who provides coordinated care throughout a patient's lifespan. Most importantly, the provision of medical homes is said to lead to better access to health care, increase satisfaction with care, and improved health!

There are many facets to implementing a PCMH, which is why the Premier HealthCare Training Institute will be hosting its ninth annual full-day conference, “The Patient Center Medical Home: Translating an Emerging National Strategy in Health Care Delivery,” on Wednesday, April 28 in New York City.

While the mission of the YAI Network and Premier HealthCare is to serve people with developmental and learning disabilities and their families, we have expanded the scope of this conference to attract national leaders of the PCMH movement. The conference will bring together top experts from places such as Dartmouth Medical School, the American College of Physicians and Healthcare Transformation IBM who will discuss the philosophy and implementation of PCMH.

The conference is relevant for professionals in the disabilities community as well as those working in health services for the general public.

Coordinated and compassionate care, based on the Medical Home Model, is a hallmark of Premier HealthCare.
