Support Coordination

Support Coordination is a division-funded service that assists people with I/DD in gaining access to needed program and State plan services, as well as needed medical, social, educational and other services. Support coordination is provided by a qualified support coordinator who uses a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan (ISP) to ensure that the person we support and/or families’ needs are always being met.

NIPD/NJ provides Support Coordination Services in Essex, Hudson, Somerset, Sussex, Union, and Warren Counties.
