Support Coordination

Support Coordinators assist people with developmental disabilities and their families to enroll in and gain access to services and supports appropriate to their needs. They assist in obtaining medical, clinical, social, educational, financial, vocational, and residential services. Support coordination is provided by a qualified support coordinator who uses a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan (ISP) to ensure that the person receiving services and/or their families’ needs are always being met.

As a provider who has been released by DDD to approve our own plans, NIPD/NJ strives to ensure that your or your loved one's service plan is completed in a quick fashion to secure the services of your choice.
