Support Coordinators assist people with developmental disabilities and their families to enroll in and gain access to services and supports appropriate to their needs. They assist in obtaining medical, clinical, social, educational, financial, vocational and residential services. Support coordination is provided by a qualified support coordinator who uses a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan (ISP) to ensure that the person receiving services and/or their families’ needs are always being met.
